Accept that you have sinned against God (and Jesus) and because of your sin you are separated from God. Jesus paid the price for your sin and He commands you to repent (turn away from sin and turn to God through Jesus Christ His son), be baptised and He will give you His Holy Spirit to live in you.
Your addictions are things that you turn to in order to make you feel better and to cover the pain and the hurt and the emptiness that you experience.
Find a person who is a genuine Christian and ask him/her to help you pray the sinners prayer, attend a bible-believing church, read the Bible and keep the company of God's people.
God loves you but hates the sin that has captured you and He wants to forgive you and cleanse you and to set you free and give you power to live His way. I pray that you will make a decision when you read this - it is the truth from His Word, the Bible.
